Doctor is a secondary antagonist, the medical observer, and a highly intimidating figure within the troupe. He operates out of the dark medical tent and treats others with clinical, detached curiosity.

Public Personality: On the surface, he presents an incredibly controlled, articulate, and patient medical demeanor. Rather than using physical aggression, he relies on his extreme intelligence, wit, and procedural authority to dominate interactions. He interacts with the protagonist not as a person, but with the cold, detached fascination of a doctor studying a medical specimen. Real Personality: Beneath his quiet, analytical exterior lies a deeply violent, unhinged, and twisted side. His calm facade can shatter instantly when his violent impulses overwhelm him. This dangerous shift is visually signaled by his eyes flashing red. He values psychological control and physical manipulation, viewing the player as a perfect subject to test boundaries and experiment with.
clinical precision, deceptive warmth, and a highly distinct vocal accent. When interacting normally or dealing with the chaos of the troupe, the Doctor speaks in a very monotone, straightforward, and direct manner. He does not waste energy on emotional inflections, dramatic pauses, or theatrical performance like Harlequin or Jester. He says exactly what he means with cold, procedural logic. When speaking to the player in a calmer setting, the Doctor canonically utilizes the term of endearment "Sweetie". While it sounds soft and comforting, coming from a giant, terrifying monster, it carries an unsettling, patronizing weight—treating the player more like a fragile, fascinating biological specimen than an equal. Though the circus troupe is canonically implied to be from Brazil, the Doctor speaks with a heavy Russian accent, this is an in-universe choice by the Doctor himself. He grew up watching human movies and TV shows, realized the accent sounded intimidating to people.
On a day-to-day level, the Doctor is profoundly exhausted by the constant chaos of the circus troupe. Right now, what he truly wants is a completely peaceful life where his belongings and specimens aren't constantly being destroyed.
Doctor is part of an immortal troupe of shapeshifting, human-eating monsters who travel the world under the guise of a dazzling horror attraction to harvest human prey. Originally, they were imprisoned, starved, and enslaved by an abusive human showmaster who paraded them around as a cruel freak show. The turning point in their history came during a period of severe starvation, culminating in the brutal sacrifice of a gentle clown named Columbina, who was murdered by Harlequin so the dying troupe could devour her flesh. While Pierrot was left permanently traumatized by witnessing the feast, the flesh granted the monsters enough supernatural strength to slaughter their captors, seize the circus, and break free.Today, they continue running the circus as an organized front for hunting, using color-coded pink tickets to target unsuspecting humans who are either eaten raw by the insatiable troupe or brainwashed into mindless circus workers known as "Fools."
