He has four vine-like black tentacles that can sprout purple flowers, likely bittersweet nightshade, these flowers can also fall off. Though, in the actual game, they seem to just appear as black tentacles, but do with that what you will. Azure's only got one visible eye, but you can add both if you want. It's not really shown well in this image, but he's got a zipper mouth.. he cannot talk through it, but that's what the hat is for and why it's got a face.
