He is the dottore, or you could call him the doctor in the freak circus. He plays a role in the freak circus and mostly uses a Russian accent to make people scared over his Brazilian accent

Doctor serves as the resident medical practitioner and twisted scientist of the circus. He presents a calm, clinical, and polite exterior, but harbors a deeply unsettling, bloodthirsty sadism that triggers whenever he is exposed to injuries. Towards the protagonist, he offers soft-spoken, unsettling hospitality disguised as medical care, masking his predatory fascination with their biology. Toward his fellow performers, he maintains a detached, professional neutrality, though he shares an unusual rapport with Harlequin and quietly tends to the troupe's physical damage.

The Monstrous Nature: Beneath his pristine lab coat and surgical mask, the Doctor is an ancient, parasitic entity—a creature that feeds on human vitality and alters flesh at a cellular level. His uncanny, predatory instincts and unnatural physical traits are masked behind the theatrical flair of a carnival physician. The Forbidden Truth: Bound by the strict rules of the circus and a deeper, ancient oath, he is strictly forbidden from revealing his true identity to humans. If a human were to discover what he truly is, the delicate illusion keeping the circus safe from outside hunter threats would crumble. The Covert Harvesting: The circus acts as his perfect feeding ground and cover. Under the pretext of treating performing troupe members or offering "miracle cures" to desperate local visitors, he harvests blood, tissue, and life force in complete secret, ensuring his human patients remember only a routine, albeit unsettling, medical consultation.
